Distinguished Service Award

 

The Distinguished Service Award (DSA) shall recognize one individual annually who has made exceptional and sustained contributions to the Society at the regional and state levels. The DSA recipient does not necessarily have to be a pharmacist (e.g., dean of a pharmacy school, legislator, or public official).

Call for Nominations and Instructions

Distinguished Service Award candidates must meet the following criteria:

At least five (5) years of extraordinary volunteer service at the state or regional levels of CSHP (not necessarily continuous), OR Activities in which the candidate has been involved have been critical in helping CSHP achieve its mission.

Volunteer activities have resulted in unusually successful and/or high quality programs, projects or services for CSHP’s members.

Individuals currently holding elected CSHP positions or those employed by the Society are ineligible.

Applications should be sent to CSHP, 1314 H Street, Suite 200, Sacramento, CA 95814. The annual deadline is March 1.

The most recent CSHP Distinguished Service Award recipient shall be provided complimentary registration, round-trip transportation and lodging as arranged by CSHP for a maximum of three nights during Seminar.
