CSHP is now offering advanced training certificate programs for pharmacy professionals. Each program was developed and will be taught by highly qualified faculty from universities and hospitals throughout California. Each certificate program employs both online and in-class instruction to provide students with both a practical and practice-based learning experience. 




The successful completion of an advanced practice pharmacist certificate program fulfills one of the three criteria required by the California State Board of Pharmacy for pharmacists to become licensed as an advanced practice pharmacist. CSHP is now offering two unique programs which meet this requirement.

Advanced Practice Pharmacist Certificate Program - Chronic Disease State Management - multiple course locations and ongoing (Now Launched!)
Presented in partnership between the California Society of Health-System Pharmacists & Touro University California

 

This program will focus on management of common chronic disease states and issues including hypertension, diabetes, hyperlipidemia, anticoagulation, depression, anxiety, and safe medication use in older adults.

A combination of home study, clinical scenarios, patient interviewing skills, and hands-on practicum sessions will ensure each participant has the skills and competency to practice as an advance practice pharmacist. Upon successful completion of this program, participants will be capable of:

  • Performing patient assessments
  • Ordering and interpreting drug therapy-related tests
  • Referring patients to other health care providers
  • Participating in the evaluation and management of disease and health conditions in collaboration with other health care providers
  • Initiating, adjusting and discontinuing drug therapy

Program Highlights:

  • Fulfills the certification requirement for advance practice pharmacist licensure
  • Earn 38 CPE credits
    • 30 hours of web-based self-study modules, including instructor-led videos

Advanced Practice Pharmacist Certificate in Comprehensive Medication Management - one time program offered at Seminar 2017
Presented in partnership between the California Society of Health-System Pharmacists & University of Southern California, School of Pharmacy

The University of Southern California School of Pharmacy Comprehensive Medication Management (CMM)-based advanced practice pharmacist certification program prepares participants to identify, stratify, evaluate, and manage high-risk patients with poorly-controlled chronic diseases. 

Program Highlights:

  • Fulfills the certification requirement for advanced pharmacist practice licensure
  • 20 hours web-based self-study modules.
  • 8 hours live classroom instruction on October 26 at CSHP 2017 Seminar in Las Vegas with patient interaction and final assessment.
  • The program involves 20 hours of online education and 8 hours of live instruction that includes motivational interviewing, active case management, and 4 OSCE (Objective Structured Clinical Examination) stations including one where each participant will provide CMM for a standardized patient. 
  • Note: Participants must attend at least one day of Seminar to participate in this certificate program.
  • $600 CSHP members | $950 non-CSHP members

Transitions of Care Certificate Program
Presented in partnership between the California Society of Health-System Pharmacists & Cedars-Sinai Medical Center

This program is designed to provide the participant with a comprehensive approach to developing a transitions of care program in their own practice setting through web-based modules and a live interactive session at the CSHP Seminar Conference.

The web-based self-study modules will provide evidence-based literature demonstrating the impact of pharmacy staff transitions of care on patient care and a stepwise approach to creation and expansion of transitions of care services integrating technicians, pharmacy students and residents. Modules are designed to be sequential and build upon the information presented in the previous session. The series also follows a patient from hospital admission through to discharge and post-discharge, and including the pharmacy staff’s roles and responsibilities at each phase of care.  Metrics to demonstrate the impact of transitions of care will also be provided.

  • Total 9 hours of continuing education units (6 hours web-based and 3 hours live)

Pharmacogenomics Certificate Program
Presented by the California Society of Health-System Pharmacists

This program has been designed specifically for health-system pharmacists to provide them with the knowledge of genetic factors that influence drug disposition, response, and adverse effects.  Program participants will gain the practical, clinical skills necessary to apply precision medicine at their own practice settings.

  • The live event will be held on October 26, Thursday from 8:00am-5:00pm at CSHP's Seminar Conference 2017.
