Dr. Jackie Ho, Pharm.D., MPH, BCPS is a Clinical Pharmacy Coordinator at San Leandro Hospital – Alameda Health System. Dr. Ho’s main practice areas include infectious diseases, medication safety and formulary management. Jackie received her PGY-1 Pharmacy Practice Residency at Kern Medical Center (Bakersfield, CA) and PGY-2 Health System Pharmacy Administration Residency at the VA Palo Alto Health Care System (Palo Alto, CA). Jackie’s professional areas of interest include outcomes research, clinical program development, population health management and infectious diseases.
During her professional career, Dr. Ho has served on the following committees: Quatra County CSHP Newsletter Editor-in-Chief (2017-present), CSHP Health Care Reform Committee (2016-present), CSHP Editorial Advisory Board (2016-present), and Pharmacy Administration Residency Collaboration Research Committee Lead (2016-2017).
Anh La, PharmD, BCPS | Chair Elect
Dr. Anh La, Pharm.D., BCPS is a Clinical Account Director for AstraZeneca and has been with the company for over fifteen years. Prior to joining the pharmaceutical industry, she was a Clinical Pharmacy Coordinator in New York City after completing her PGY-1 Pharmacy Practice Residency at the Bronx Veterans Affairs Hospital. She has represented AstraZeneca Medical Affairs for numerous therapeutic areas including oncology, cardiometabolic, and neuroscience. She received her Pharm.D. degree from the University of the Pacific and returned to California, now residing in San Diego with her husband and two elementary-aged sons.
Becky Natali, PharmD, MS, BCPS | Member
Dr. Becky Natali, Pharm.D., MS, BCPS is the Director of Clinical Affairs and Clinical Pharmacist at the Beverly Hills Cancer Center. Prior to her current role, Becky worked as the Assistant Director of Pharmacy at Santa Monica- UCLA Medical Center & Orthopaedic Hospital and a Pharmacy Director. Becky completed her Doctor of Pharmacy and Master of Science in Pharmacy Administration from the University of Houston and completed a PGY-1 and PGY-2 in Health System Pharmacy Administration with the Houston Methodist Hospital. Her professional interests include: Oncology, Clinical Research, Sterile Compounding, Medication Safety, Informatics, Pharmacy Leadership, Regulatory Affairs and Finance. Outside of her career she loves to travel, spend time with her family, surf and hike with her dog.

Donald Kishi, PharmD | Member
Dr. Don Kishi graduated from the UCSF School of Pharmacy, Class ’68 and was selected to be a member of the 9th Floor Pharmacy Satellite Pilot Program at the UCSFMC. He has served both the UCSF Department of Pharmaceutical Services and the Division/Department of Clinical Pharmacy in the School of Pharmacy as a clinician, preceptor, educator, and researcher and in various administrative capacities. He is currently an Associate Dean in the School of Pharmacy. He has also served on the boards and as a presidential officer of the UCSF Pharmacy Alumni Association (PAA) and of the Campus Alumni Association (AAUCSF). His service to CSHP includes various councils, committees and task forces, seminar management team, board of directors, and presidential officer. He is currently on the CSHP Foundation’s Board of Directors. He has been recognized as a CSHP Fellow, Pharmacist of the Year and Distinguished Service Award recipient.
He also served on the ASHP Board of Directors. With respect to the HRC, it is an ideal time for pharmacy to define needs and document, fill, evaluate and promote them.
Pierre S. Del Prato, PharmD, MS, RPh (Retired) | Member
Dr. Pierre Del Prato has been an active member of the CSHP Health Care Reform Committee since its inception and continued his interest in the dissemination of accurate, quality information on cutting edge subjects. Are of personal focus is Addiction Pharmacy – identification, intervention, treatment and recovery. Pierre is pleased to be a member of the CSHP Opioid Stewardship Task Force.
Pierre received his Pharm.D. at USC and MS and two years of Clinical Hospital Pharmacy Residency at The Ohio State University Hospitals and Clinics. Pierre has been a staff pharmacist at the Presbyterian Inter-community Hospital (Whitter, CA), Valley Presbyterian Hospital (Van Nuys, CA), and Mercy General Hospital (Sacramento, CA). Pierre has also assumed various leadership roles and has been the Assistant Director of Pharmacy at St. Vincent’s Hospital (Los Angeles, CA), Direct of Pharmacy at Charter Hospital (Roseville, CA), Direct for Drug Interactions Project and Director of Clinical Practices APhA (Washington, DC), Director, Pharmacy Benefit Program, United Mine Workers of America Health & Retirement Funds (Washington, DC), Vice President, Education CPh A (Sacramento, CA) and the Executive Director, CPhA Educational Foundation (Sacramento, CA).
Annie Xu, PharmD | New Practitioner Member
Dr. Annie Xu is currently a PGY1 resident at Dignity Health Mercy General Hospital. She completed her pharmacy school training at Touro University in Vallejo, California. Prior to beginning her career in pharmacy, she obtained a BS in Social Sciences at the University of California, Irvine. Her main goal as a pharmacist is to advocate for the underserved population and provide quality care for all patients. Currently, her professional areas of interest include informatics, critical care, and infectious diseases.

Inpatient Pharmacy is a specialty that my career has encompassed for the last 19 years. My initial licensing and certification began in 2000. The Santa Rosa Memorial Hospital gave me 16 years of unparalleled experience as an Inpatient Pharmacy Technician. Today, I work in this capacity at Highland Hospital, Alameda Health System.
For the last three years I've volunteered with: CSHP Health Care Reform Committee, D.S.H.P. Diabetes Outreach Health Fair Event, and Co-Chair Night Out With Industry for D.S.H.P..
After receiving my Certified Nursing Assistant License in 2017, I take any opportunities to care for individuals and my family as a Home Health Aide.
Leadership, duty, respect, selfless service, honor, integrity, and personal courage are the core values that were embedded in my training as soldier in the U.S. Army. Through my 4 years serving with one year of experience overseas in South Korea, I worked as a Light Wheeled Vehicle Mechanic. This civic service gave me confidence in my ability to figure out how things work, as well as the ability to follow orders. The knowledge gained has been instrumental to my success in the medical field.
Learning is a continuous process that will never stop. I look forward to the challenges in the chapters to come.
Sylvia Banzon, CPhT, CQIA, HACP, PMP, FCSHP | Pharmacy Technician Member
Sylvia Banzon, CPhT, CQIA, HACP, PMP, FCSHP is the Integrated Quality Services Manager at Sutter Coast Hospital in Crescent City, CA. She is responsible for the Accreditation and Licensing, Cancer Care, Emergency Management, Patient Safety, Pharmacy, Quality, Performance Improvement and Risk Management Programs. Sylvia has worked as a Pharmacy Technician for over 20 years. Her last position in Pharmacy was as the Pharmacy Technician Supervisor/Project Coordinator at Sutter Medical Center Sacramento.
Sylvia has been a member of CSHP for over 19 years and has served on various committees and task forces. She was the first technician on the CSHP Board of Directors; served as Chair of the Technician Division Executive Committee (TDEC) and President of the California Pharmacy Technician Association (CPTA). She has been recognized as a CSHP Fellow and received the Technician Achievement Award in 2012. She is currently serving on the CSHP Healthcare Reform Committee.
Sylvia has served on the ASHP CE Committee and Section of Inpatient Practitioners- Pharmacy Support Services. She has been involved with PTCB as Subject Matter Expert for the Pharmacy Technician Certification Exam (PTCE) and was the keynote speaker for the PTCB Crest Summit. Sylvia has also participated with the California Community College Economic and Workforce Development Program Health Initiative to update training and education programs for pharmacy technicians in California.
Katayoon Kathy Ghomeshi, PharmD, MBA, BCPS, CPPS | Member
Katayoon Kathy Ghomeshi, PharmD, MBA, BCPS, CPPS is a Medication Safety Officer at UCSF Medical Center and Health Sciences Assistant Clinical Professor at UCSF School of Pharmacy. Dr. Ghomeshi serves on the California Society of Health-System Pharmacists Healthcare Reform Committee and Council on Professional Affairs. She is an appointed member of the California Hospital Association Medication Safety Committee. Dr. Ghomeshi is the Vice Chair of the American Society of Health-System Pharmacists Section of Inpatient Care Practitioners’ Advisory Group on Medication Safety. She is also an appointed Specialty Council Member for the Board of Pharmacy Specialties.
Dr. Ghomeshi earned a Bachelor of Science in Neurobiology, Physiology, and Behavior from the University of California, Davis. She completed a Doctor of Pharmacy degree from the University of Maryland, Baltimore School of Pharmacy. She earned a Master of Business Administration from the University of Baltimore/Towson University. She completed a PGY1 at Kaiser Permanente/Kaiser Foundation Hospitals, and a PGY2 Medication-Use Safety Residency at the Johns Hopkins Hospital. Dr. Ghomeshi is a Board Certified Pharmacotherapy Specialist, as well as a Board Certified Professional in Patient Safety. She is passionate about improving the safety and quality of pharmaceutical care, promoting a culture of safety, evaluating human factors, and developing effective error and risk reduction strategies for medication use.
Julia K. Nguyen, PharmD, BCPS, BCGP | Member
Julia K. Nguyen is an assistant clinical professor at University California San Francisco School of Pharmacy. Her current practice site is at Kaiser Permanente Panorama City Medical Center where she is a pharmacist specialist in outpatient infusion pharmacy services including home health, palliative care, hospice, home infusion, clinic administered medications, and skilled nursing facilities. At her medical center, she serves as chair and reviewer of Palliative/Hospice Continuing Medical Education (CME) and is engaged in active research with medication use and health policy.
In her teaching role, she serves as a PGY-1 preceptor and has been recognized for outstanding clinical teaching multiple times with the Kaiser Permanente Residency Preceptor of the Year Award and University of California San Francisco Apples for Preceptor Award. As a previous recipient of Kaiser Permanente National Pharmacy Program & Services Performance Improvement Triple Aims Awards, she currently is mentoring skilled nursing facility antimicrobial stewardship and innovative pharmacy practice projects.
Prior to her current role, she earned her Pharm.D. from University California San Francisco School of Pharmacy in 2003 and completed a PGY-1 pharmacy practice residency program at Kaiser Permanente Woodland Hills Medical Center. She is currently board certified in both pharmacotherapy and geriatrics and is licensed as an Advanced Practice Pharmacist.
During her professional career, Dr. Nguyen has served in the following leadership capacities: CSHP Government Affairs Advisory Committee (2017-2018), CSHP Committee on Nominations (2009-2010), and CSHP San Fernando Valley Chapter Delegate (2005-2007). She received the San Fernando Valley Chapter Pharmacist of the Year Award in 2008.
Kofi Asante, PharmD | New Practitioner Member
Kofi Asante, PharmD, recently completed a PGY-2 resident in pharmacy administration at Kaiser Permanente in Downey, CA. He is a Brooklyn, New York native who received a B.A. in biology from the University of Rochester. After graduation, he then taught honors biology at Lafayette High School in Brooklyn, NY. He obtained a Master’s degree in biology at the State University of New York at Albany and worked in the cardiovascular research department at Albany Medical Center, capturing the hearts of all. He earned his Doctor of Pharmacy degree from D’Youville College of Pharmacy in New York. He relocated with his wife and two sons to Southern California in 2013.
As a pharmacy administration resident, Kofi explored effective leadership and management styles, and components that contribute to pharmacy operational excellence at Kaiser Permanente. Additionally, he focused on improving quality outcomes and affordability for home infusion services. Prior to his pharmacy administration residency, Kofi completed his PGY-1 training in managed care at CareOregon in Portland, OR. Focusing on formulary management, Kofi developed cost-saving and medication adherence initiatives to ensure appropriate medication usage and deliver optimal pharmaceutical care to the Portland community. Currently, Kofi is a pharmacist at Torrance Memorial Medical Center, and continues to pursue his goal to guide a team and make pharmacy operations simple and efficient.

Quang Vu is a third-year Pharm.D. candidate at Loma Linda University School of Pharmacy, Class of 2020. His professional interests include: Cardiology and Critical Care.
