Affiliate Chapter of Distinction Award

The Affiliate Chapter of Distinction Award was established to recognize chapters that show excellence in finance, business processes, membership growth, and member satisfaction.

Note: This award has been updated as of March 19, 2020. All submissions for this award after the mentioned date must use the new application and uphold the new criteria. Please refer to the application for more details.



  • Central Valley

  • Golden Gate
  • Inland
  • Orange County
  • Quatra County
  • Sacramento Valley
  • San Fernando Valley

  • South Bay Long Beach
  • Affiliate chapters applying for recognition must supply evidence of adherence to at least eighty percent (80%) of established criteria
  • The CSHP office shall notify chapters gaining recognition status at or before the Winter Chapter President's Meeting
  • Affiliate Chapters of Distinction will be permitted to use their designation on any chapter-related materials


  • Provides the names, board positions, and email addresses to CSHP prior to your installation ceremony event
  • Provides the names, credentials, email addresses, and telephone numbers of your delegates to the House of Delegates by August 1.


  • Establishes an annual budget
  • Conducts an annual audit
  • Files tax returns with California (i.e., Form 199n) and the federal government (i.e., Form 990-N) annually

Business Processes:

  • Attends at least seventy-five percent (75%) of CSHP webinars offered to affiliate chapters
  • Attends all chapter presidents meetings
  • Conducts an annual election of officers
  • Seats full slate of delegates at HOD
  • Seventy-five percent (75%) of chapter delegates and alternate delegates attend RDC, with the exception that it shall be 50% for chapters with only two allotted delegates
  • Attends the Open Forum at Seminar
  • Submits quarterly reports (such as membership recruitment activity, member services, business activity, etc.) to CSHP
  • Submits meeting minutes no less than quarterly to CSHP
  • Provides updated contact lists of hospital and health-system pharmacies and directors


  • As of September 30th of the award year, records membership retention of at least ninety percent (90%) of membership levels on September 30th of the previous year
  • Recruits at least ten percent (10%) new members annually based on membership levels on September 30th of the previous year

Membership Satisfaction:

  • Publishes a quarterly newsletter OR maintains a website updated quarterly
  • Holds a minimum of four (4) continuing education (CE) events per year (co-hosted/co-sponsored events qualify as meeting the intent of this criterion)
  • Conducts or participates in at least one (1) community service project annually (e.g., DEA drug take-back days, health fairs, brown-bag events) annually
  • Submits at least one candidate nomination to CSHP Board of Directors and/or a CSHP committee
  • Submits at least one candidate nomination for the CSHP Pharmacist/Pharmacy Technician Fellow Award Program
  • Submits one (only) candidate nomination for the Affiliate Chapter Volunteer of the Year Award
  • Participates in legislative advocacy such as : holds at least one legislative awareness event; has at least one member attend CSHP legislative day; meets with local representative with at least two (2) chapter members

The annual deadline for submissions is November 1st


The recipient chapters of the Affiliate Chapter of Distinction Award will receive the following:

  • Certificates of Excellence at the Winter Chapter President's meeting
  • Award recipient publication notice in CSHP's weekly newsletter, InfoSource
  • Recognition at CSHP's annual award ceremony at Seminar
  • Perpetual name plate distinction added to the Affiliate Chapter of Distinction Award plaque
  • One full early-bird Seminar registration for the chapter member of your choice
