STAFF PHARMACIST – AMBULATORY CARE

Santa Clara County, California

The County of Santa Clara Health System (CSCHS) is currently seeking to fill one extra-help Staff Pharmacist position in the Ambulatory Care division.

The Pharmacist in the Ambulatory Care Division of the Pharmacy Department will initially spend time assisting with patient enrollment in ongoing research studies at the Anticoagulation Clinic. In the future, it is anticipated that the extra-help pharmacist will backfill at other CSCHS ambulatory care clinics.

Minimum qualifications include possession of a valid CA Pharmacist license, and past experience (APPE or residency rotation) in an anticoagulation clinic. Residency training is preferred, but not required. Also, availability to work at least 16 hours per week is a requirement.

The ideal candidate will have a working knowledge of warfarin and direct oral anticoagulants, excellent oral communication skills, basic computer skills (excel, word, powerpoint, etc.), and show the ability to work well in a team with other disciplines in an interdisciplinary environment.
